Now that you're on your way to becoming part of the HDS family, there are a few things we should all know, and have written down!

What are your pets names, species and or breeds?

Do your pets have any known allergies to certain indoor or outdoor substances?

Is there a certain behavior you are trying to keep your pets from contnuing?

Are there things that frighten or trigger your pet?

Are there areas of your house that your pets are not allowed?

Are there any injuries or permanent issues with your pet? (Limping, favoring a leg, etc...)

Have your pets ever shown any aggression towards people or animals, and in what scenarios?

What is the typical amount of time your pets are used to being left alone when you need to leave your house? Is this the duration of time alone you are ok with them being kept to?

If am walking a pet for you, are there certain commands you like used? And is there leash behavior I should know about? i.e., pulling, reactivity to other animals or people, aversion to certain actions... (reaching for collar, being leashed, being touched).

Are your pets destructive when left alone?

YES

If so, how do you handle these destructive behaviors and prevent them?... crates, kennels, shut in a room...etc

Who is your veterinarian, and where are they located? Please remember to fill out the veterinarian care release form

Local emergency contact for pet related issues. Is there someone you'd like me to contact if your pet has an emergency? Please list their, name, conatct info, and who they are to you.

What time of day are your pets used to eating their meals? Please list other times of day your pets are used to certain activities... (potty breaks, walks...etc).

Do you have any other details you'd like me to know about your animals?
